The Health Information Management Department at North Cypress Medical Center is dedicated to safely maintaining your medical records and keeping your health information private.
Copies of your own medical records, those of a minor child, or the medical records of someone for whom you are the legal guardian or hold a Durable Power of Attorney for Healthcare may be requested. Proof of legal guardianship or a Durable Power of Attorney for Healthcare is required for access of the records.
To obtain copies of your medical record, contact our Health Information Management Department (Medical Records Department) at 832-912-3530 during regular business hours, Monday - Friday from 8 a.m. to 5 p.m. To obtain a copy of your medical records, please complete our online form. Please complete the form and deliver it to the North Cypress Medical Records Department or you can mail to the following address:

Due to confidentiality reason we are unable to fax medical records to your home or office. Please be aware that there may be a charge for your records. You will want to call and speak with one of our representatives prior to mailing or bringing in your request form.
